Make sure you do not have anything interfering with the impeller by removing the powerhead, opening it, and pulling out the impeller for inspection. Clean out any sand, debris, sludge, algae, hard water deposits, etc. Use hot water and vinegar to clean the impeller, and swab out the inside of the powerhead. This should prevent any more issues, but if you have sand as your substrate, make sure the powerhead is at the top of the tank or put some floss inside the little strainer to keep sand out.
